Below are the forms you will need to fill out with interview participants if you wish to conduct your own interviews for our Bridging the Gap initiative. Consent forms ensure that the people you interview understand how their interviews will be used, and consent to their publication on our website. Remember that  interviewees can elect to end their participation at any time before or during their interview, and that they can elect to release all or part of their interview after the interview has taken place using the provided release form.
